Our Project HOST Program
Project HOST stands for Healthy Outcomes and Supported Transitions. This program assists families and individuals, primarily in Albany and Rensselaer counties, needing affordable housing with tenancy support through case management and advocacy services.
In conjunction with Connect, Project HOST also offers supportive housing and case management services for homeless singles. These programs aim to help people who face a variety of complex challenges, live with stability, autonomy and dignity. Project HOST works to improve the health and well-being of people experiencing homelessness or housing instability.
Services Provided
House Case Management
- Service plan development
- Coordination with care management & hospitals
- Transportation to appointments
- Entitlement assistance
- Support groups
- Supported employment services
Tenancy Supports
- Housing application and search assistance
- Furniture acquisition
- Understanding tenancy rights and responsibilities
- Managing landlord relationships
- Entitlement
Eviction Prevention
- Paying rent on time
- Conflict resolution
Foster Care
- Rental subsidy: assistance with rent payments to promote independence
Program Eligibility
To be eligible for these services, our team will evaluate Medicaid eligibility, health status, housing status, a Vulnerability Index Score, and if the applicant has been a recent, frequent user of emergency services in the past six months.
